Missouri

COVID vaccines administered at Missouri pharmacies are available to all Missouri residents 16 years and older.

For those aged 16-17 years, a parent or guardian must be present to sign the vaccine consent form. Please note that only the Pfizer vaccine is approved for ages 16-17 years – please schedule only at sites that are labeled with Pfizer.

When making an appointment, you will need to attest that you or the person that you are making the appointment for meets those criteria. If you meet other criteria for the current state phase, please visit the website of your local health department and/or health system for vaccine appointment.

When appointments are available, they will be active in the yellow drop down menu above. Once each location has hit their maximum number of appointments for the week, when you click on the link, you will see an error message stating that there are no more appointments available for that week.

When you complete the form to make an appointment, you will see a confirmation page with a green checkmark. You will also receive a confirmation email to the email address you entered into the form - please double check that your email is entered correctly! If you do not see either the green check or the confirmation email, you do not yet have an appointment - please try the form again. We are also aware of a limited error where a confirmation email is received without an appointment date/time listed - if this happens, you do have an appointment but it may not be at the time you had selected. Please wait for the pharmacy to contact you at the phone number given in the form.

Frequently Asked Questions

Q: What type of vaccine is being administered?
A: The vaccine type will be listed in the appointment form. Some pharmacy locations will have Johnson and Johnson, others will have Moderna. Moderna is a two dose vaccine, so the second dose appointment will be exactly 28 days out from the first date.

At the location listed as Price Chopper Vaccine Clinic, which is located at 75th and Metcalf, we are only administering the Pfizer vaccine, so second doses of vaccines given at that clinic site will be 21 days out from the first dose.

Q: When will my Balls Food Pharmacy get the COVID vaccine in stock?
A: All of our pharmacies are currently receiving limited doses on a weekly basis, but this is based on federal and state allocation and is not guaranteed. When dose arrivals are confirmed, we will make new appointments available in the system, so please continue to watch the website for appointments.

Q: Who can get their vaccine from a Balls Foods Pharmacy?
A: Our pharmacies are required to follow guidelines from local and state health departments to decide who to vaccinate. When you make an appointment for a COVID vaccine, you will need to confirm that you are in the current phase, which will be described in the appointment system. For information on who is currently eligible for the COVID vaccine in each state, please visit:

Q: What information will I need to schedule an appointment?
A: You will need to enter the following information into our appointment scheduler: Name, address, date of birth, phone number, and email.

Q: What do I need to do on the day of my appointment?
A: Please bring your driver’s license or state ID and your insurance card, including your red, white, and blue Medicare Part B card if you have one. You will need to stay in our building for 15 to 30 minutes after each dose, to make sure that you do not have an allergic reaction.

Q: How do I schedule my second dose?
A: When you make your first dose appointment, your second dose appointment will be confirmed for 21-28 days out from your first dose. For example, if your first dose is Monday at 2pm, your second dose will be 3-4 weeks out for Monday at 2pm. You will receive your vaccination record card with your second dose appointment date on the day of your vaccination.

Q: How do I cancel an appointment?
A: Please go to our Cancellation Request Form and submit your cancellation.
